Free Tax Prep

Required

• Photo ID 
• Social security cards for you, your spouse, and dependents (SSA-1099 are acceptable)
• Proof of bank routing numbers and account numbers for Direct Deposit, such as a bank check or savings card (not a deposit slip)
• Income and expense documents 

Income Document Types 

• Wages and earning statements, Form W-2, 1099-R, from all employers
• Other income statements: Form 1099-G, W-2G, 1099-MISC
• Interest and dividend statements from banks (Forms 1099 INT, B and D)
• A copy of last year's federal and state returns, if available, or your Adjusted Gross Income (AGI) from last year

Expense Documents Types

• Total paid for daycare provider and daycare provider's tax identifying number (SSN or EIN)
• Form 1095-A if you purchased Health Insurance from healthcare.gov, or if the government paid part of your health insurance premium

Please note:

• To file taxes electronically on a married-filing joint tax return, both spouses must be present to sign required forms at pick up
• All clients will be asked to sign Form 14446
• You will need to come twice: to drop off forms and to pick up of returns and signs
• Preparation can take 1 - 3 weeks
